Safeguarding Outside YoUR GP Program

As one client recently said “this was the first winter which I was not admitted to hospital as a result of transitioning to HyCare”, a marker of a good service provider is one with programs in place to keep clients safe. At HyCare, our safeguarding program is being able to speak to a doctor for medical advice with the aim of hospital avoidance

Carer Training and Accreditation

As one client puts it “my carers are an extension of my eyes, ears, arms, and legs”. At HyCare, we understand “Duty of Care” and recognise that providing safe supports and hospital avoidance is about providing support to our carers: both through accrediting them with the skillsets that they need to help you, and at the time that they need the help, whether that be at 2pm or 2am

Transportation

At HyCare, transportation is not to treat a passenger as cargo, nor simply about taking someone from one place to another. Every journey is an opportunity to support independence, build confidence, strengthen community connections, and create meaningful experiences

  • 24/7 Clinical Support: round-the-clock medical advice via escalation care plans, collaborating with clients' GPs for after-hours clinical safeguarding.
  • Free Health Programs: participants gain free access to integrated internal programs, including preventative care, telehealth, dental, and general health checks.
HyCare will organise a meeting as soon as you call us and discuss what services would be required. Services can commence as soon as we know how we can best support you and have a signed service agreement.
